The PCN hub is based in Wadebridge and is made up of three practices in North Cornwall, Wadebridge and Camel Practice, Port Isaac Practice and Bottreaux based in Boscastle. Social Prescriber Link Worker

Job Types: Permanent, Full-time

An exciting opportunity has arisen to join North Cornwall Coast PCN to work with 3 GP practices, Bottreaux Surgery, Port Isaac and Wadebridge and Camel Estuary as a Social Prescribing Link Worker. We aim to improve health and empower people and communities to live well by social prescribing provision to our patients in the local area. Taking a holistic approach, you will connect people to community groups and statutory services to provide them with practical and emotional support.

Dispenser

We are looking for a cheerful, enthusiastic dispenser(s) to join our busy, friendly GP surgery.

A minimum qualification of NVQ Level 2 or 3 in Pharmacy Services is required. Must have excellent communication skills, a confident telephone manner is essential and excellent customer service skills, good IT knowledge and ideally with some experience of primary care.

This permanent position is for 37 hours a week, part time/job share considered.

Paramedic/Advanced Practitioner

We are looking for an enthusiastic, innovative and forward thinking paramedic/advanced practitioner to work with our network of GP Practices in North Cornwall. We see our PCN paramedics as key clinicians in delivering excellence in patient health care; and to help support our GP practices with a model of ‘on the day’ care providing clinical care (including telephone triage, diagnosis, treatment, referral, review or discharge) as appropriate, utilising a range of advanced skills and expert knowledge for patients presenting with undifferentiated and undiagnosed conditions

If you are looking for a new challenge where you see and support patients with their healthcare needs, as an integral part of a GP practice, and can develop our PCN team then this role is for you!

We will provide you with professional mentoring and bespoke coaching support so you can expand your clinical skills and confidence to best support your patients. Ideally you will have a prescribing qualification and have undertaken post graduate clinical learning however this is not essential. Key to this role is an innovative driven person that wants to develop into an effective clinician.

Care Co-Ordinator

North Cornwall Coast PCN are seeking an enthusiastic and proactive Care Co-Ordinator to join our team. We are a patient focused forward thinking PCN looking for people to work with the PCN team to deliver care to their patient populations. The Care Co-Ordinator role is an evolving role within NHS Primary Care Networks. The post holder will require a flexible approach to work, the role will consist of some focussed project work as well as general core support to the PCN. The post holders would ideally have good knowledge of community or primary care sector and good IT skills.

Pharmacy Technician

We are seeking a registered, skilled and enthusiastic pharmacy technician to work in North Cornwall Coast Primary Care Network practices, to deliver audits, medicines reconciliation and general medicines optimisation tasks with all training and support provided. You will work under the supervision of a pharmacist or GP within our team and we will fully support a transition into the role – perfect for a registered pharm tech with a community background and a passion to develop clinically.

Opportunities to develop dispensary based work into the role will be supported

Full-time or part time will be considered.

Clinical Pharmacist

North Cornwall Coast PCN are looking to recruit a clinical pharmacist. You may wish to work as a lead Pharmacist or be a newly qualified clinical pharmacist looking to join our enthusiastic and proactive pharmacy team.

Within the pharmacy team you will work alongside our current clinical pharmacist, and pharmacy technician. We have excellent working relationships with all three member practices and agreed ways of working across the team.

Competitive pay, dependent on experience.

This role is primarily based at Bridge Surgery, Wadebridge but may involve working at Port Isaac and Bottreaux surgeries.
